How to Report a Claim

The NAICO claims procedures have been designed to eliminate red tape and delays. To be as efficient as possible, we must obtain information from you. To report a claim, select the appropriate forms, complete them electronically and save to your computer. You may then attach the forms to an email to claims@naico.com. Or print and fax the forms to 405.258.1329.

Worker's Compensation State Specific Forms
To report your Worker's Compensation claim, please choose the appropriate state forms below, or if the state isn't listed, use the "All Other States" form. Please read the form instructions carefully, then complete the forms electronically, save to your computer and attach to an email to wcclaims@naico.com. You may print and fax the forms to 405.258.3005.

Your claims representative will need as much information as you can provide. No detail is too small. A claim representative will first make sure you have a valid policy in effect, then your policy language will be examined to determine applicable coverage. NAICO will protect your interests in accordance with your policy provisions.

You may also report claims via phone by calling 800.332.2210. To expedite the claim process, have these items available:

  • Policy Number
  • Named Insured
  • Type of Claim and Detailed Description of Claim
  • Dates, times, who was involved, locations, witness names and contact information, detailed description of accident (no detail is too small)
  • Claim Data
  • Receipts, repair bills, medical bills, repair estimates, other relevant reports.

When reporting a claim, you will be asked to explain what happened. Try to be as accurate as possible with regards to dates, times and persons involved. Be prepared to supply witnesses’ names and telephone numbers. NAICO may require your assistance by asking for additional information (statement of the facts, receipts, repair estimates, medical reports, etc.).  Every effort will be made to process your claim in the shortest time possible.
